Club Survey

Please forward the information below to all Clubs in your Region and, if necessary, translate to your local language. The survey is available in English, Chinese, Danish, Japanese, Korean, Portuguese and Spanish.

This is the Club Survey for Towards 2022 and should be completed once by each Club before 30 April 2012. We ask for the support of Regional Directors and Area Presidents to ensure we receive as many responses as possible.

We are requesting your assistance once again this year to complete the Towards 2022 survey to assess our progress on achieving the TW22 goals.

Each club should only complete the survey once. Once you begin the survey, all questions should be answered and the survey should be fully completed in one single session.

The Club President should complete this task unless the President specifically designates the task to another person.
